Puppy Application

Please answer all questions honestly and as thoroughly as possible. This helps me know what type of information I need to give my new dog owners to help make their home a safe and happy place for my pups. After reviewing the application, I will contact you via phone and ask any additional questions I feel are necessary. I do not ship puppies, so if you cannot travel to Northwest Indiana DO NOT APPLY. Do not send deposits unless I have contacted you and have approved you as a new puppy home. I reserve the right to deny a new home if I for any reason feel that it will not be a good fit for a new puppy. I do not base my decisions on race, sexual orientation, political affiliation, or religion.
